HOW TO APPLY

Getting compensated for the costs associated with overcoming your victimization requires a few steps. You can complete these steps yourself, or receive help from your local law enforcement agency, State Attorney’s Offices, or victim services center.

Instructions to Apply

1. Fill out the Victim Compensation Claim Form, sign and date your signature.
2. Attach acceptable proof of crime, such as the incident report from law enforcement or other proper authorities.
3. Attach any supplemental documentation required as stated within each section of the Bureau of Victim Compensation Claim Form, which depends on the benefit type requested. For relocation benefits, certification of immediate need, reasonable fear, or urgent assistance must accompany the application and can only be provided by a certified domestic violence or rape crisis center in Florida. A standard housing contract or a Notification of Residential Agreement is also required at the time of application.
4. Submit the completed claim form and all required documentation via email, fax, or mail to the Bureau of Victim Compensation.
5. If you change your mailing address, phone numbers, or email, you must provide notice to the Bureau of Victim Compensation to prevent delays in processing your claim or benefits.

Upon receipt of the application, the Bureau of Victim Compensation will generate a claim number and provide you with written notification so that you can record your claim number on all itemized invoices submitted for payment consideration. If the claim is determined eligible, you will be notified to submit compensable expenses, or you may submit them with your application. Depending on collateral sources which may exist, you may also qualify for an insurance waiver provision so that you are not charged for any copays or deductibles applicable to certain insurance carriers. If the claim is denied, you will have the opportunity to appeal the determination and present material fact to substantiate the claim pursuant to law.
